本校為香港道教聯合會屬下第八間政府津貼小學，創辦於一九九六年九月，由本校校監陳國超太平紳士捐款興辦，以紀念其母親陳呂重德女士， 故定名為「香港道教聯合會陳呂重德紀念學校」。 由二零零七年九月起，本校校董會為感謝圓玄學院慷慨捐鉅款，以改善校內教學設施，特向教育局申請將校名更改為「香港道教聯合會圓玄學院陳呂重德紀念學校」。
